Inova Health Systems Juniper Program is Northern Virginia's largest provider of HIV/AIDS care. We are looking for an experienced RN Case Manager to join our team at the Inova Juniper Program - Herndon location. This is a Full-Time, Monday-Friday position (40 hours/week).*Inova Juniper Program* is proud to offer all aspects of primary medical care and mental health, substance abuse and case management services to HIV positive individuals along with a history of delivering compassionate HIV/AIDS clinical and education services.Our convenient services are available at locations around Northern Virginia. We provide compassionate, dignified care to individuals regardless of their sex, race, background, sexual orientation, religious preference, financial status, or insurance coverage. At any given time, Inova Juniper provides services to about 1,800 active clients.*Job Responsibilities:** Provides coordination of services and acts as a key Liaison between patients, families and the interdisciplinary healthcare team members.* Uses utilization management techniques to determine the medical necessity, appropriateness and efficiency of the use of healthcare services, procedures and facilities.* Discusses payer criteria and issues on a case by case basis with clinical staff and follows-up to resolve problems with payers as needed.* Identifies at risk populations by using approved screening tools and following established reporting procedures.* Participates in the assessment of patients' clinical and psychosocial needs through review of patient information, personal contact with patients/families and interdisciplinary healthcare team members.*Featured Benefits:** Tuition reimbursement* Student loan assistance up to $10,000!* Paid parental leave for birth/adoption of a child* State-of-the-art medical and training facilities* Nursing professional development support and countless opportunities for career growth*Additional Requirements:** *Education:* BSN.
