About Us Our organization has been at the forefront of establishing
a national in-home clinical network, providing comprehensive health
assessments to individuals across the country. We are dedicated to
delivering exceptional, personalized care that enhances health,
reduces costs, and empowers individuals to lead fulfilling lives.
With a strong foundation in clinical assessment and care
management, our nationwide network of clinicians transcends
traditional barriers by meeting members in their own environments.
We focus on improving the quality of care for older adults and
other at-risk populations, ensuring better health outcomes and
identifying chronic conditions that might otherwise remain
undetected. Joining our team means utilizing your skills and
compassion to make a significant impact on the health and
well-being of others. We offer competitive base salaries along with
incentive compensation and benefits, all while contributing
positively to the community! ResponsibilitiesAbout the Role During
visits that may last up to one hour, our providers assess and
observe a member’s health status, medical history, medication
adherence, social circumstances, and other potential risks. This
approach provides invaluable insights into a member’s overall
health that are often missed during standard office visits. The
Matrix Comprehensive Health Assessment (CHA) enhances care quality
and allows us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, including medical history,
diagnosis, treatment, health education, physician referrals, case
management referrals, follow-up, and thorough documentation in line
with our guidelines and protocols. Administering and ordering
necessary point-of-care and lab-based tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as appropriate. Working
with Primary Care Physicians (PCPs) on patient education and
necessary follow-ups. Providing services at the member’s residence,
which may include their homes or Skilled Nursing Facilities.
Benefits of Working with Us Consistency: While assessments are
conducted from Sunday to Saturday between 8 am and 7 pm, our
full-time providers typically work Monday to Friday from 8:00 am to
5:30 pm. Competitive Pay: Enjoy an annual base salary ranging from
$95,500 to $158,400, determined by market analysis and experience.
Incentive Program: Participate in monthly (uncapped) and year-end
incentive opportunities. Comprehensive Benefits: Receive mileage
reimbursement, medical, dental, vision, life insurance, PTO
accrual, 401(k), paid holidays, continuing education
time/reimbursement, and employee referral bonuses. Education and
Support: We prioritize your growth! Along with thorough onboarding
training, you will be assigned a preceptor, receive personalized
feedback, and gain free access to online continuing education.
Additional Licensure: If business needs arise, we will assist you
in obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. QualificationsSkills & Experience That Shine
