The role is vital for providing patient-centered, customized therapy care plans to help patients achieve optimum functional status in their home environment. Our team is driven by a shared passion for enhancing patient autonomy and quality of life through innovative therapeutic strategies and interventions. Conduct patient assessments to determine their functional status and develop appropriate therapy goals. Implement personalized therapy interventions and document patient progress. Educate patients and families on recovery techniques and home safety. Collaborate with a multidisciplinary team to provide integrated patient care. Adjust therapy plans based on patient progress and feedback. Advanced knowledge of therapeutic practices and principles. Excellent interpersonal and communication skills. Ability to handle physical demands of patient care. Strong organizational and time management skills. Proficiency in electronic health record systems. Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid state Occupational Therapist license. NBCOT Certification preferred. CPR certification required.
