Job Description Job Description Roles and Responsibilities: Management of On-Call nursing calendar and scheduling Maintains annual competencies of all nurses Collaborates with VP of Nursing to write and execute annual evaluations to nursing staff on performance and goals Conducts one-on-one meetings with individual nurses to discuss ongoing performance and department goals Uses the nursing process to care for and/or educate individuals in the promotion and maintenance of health. Uses systematic critical-thinking to guide data gathering, assessment, nursing judgment. Responsible for coordinating and managing aspects of infusion nursing care as ordered by the physician. Coordinates and/or Delivers care in collaboration with the patient, their family and all members of the health care team. Submits required documentation of care within required time frame including initial assessments, visit encounters, patient education. Manages interdisciplinary collaboration and communicates urgent clinical findings in a timely manner. Maintains privacy, security and confidentiality of patient, staff and organizational data Fosters a non-discriminatory work environment in which care and services are delivered in a sociocultural sensitive manner. Ability to work autonomously, while demonstrating strong work ethics to meet goals and provide stellar customer service to patients, MD offices and multidisciplinary team members. Travel to assigned territory as needed with willing flexibility to meet patient and/or team needs. Display neat, clean, professional appearance and demeanor at all times. Perform patient education and training on administration of their therapy treatment and use of associated equipment when needed. Perform or coordinate infusion nursing care in the patient home and/or suite/center (except where not permitted by state regulations) when the patient's home health agency or ContinuumRx Infusion Nursing staff are unavailable. Coordinate patient treatment with Home Health Agency and other ancillary service providers. Facilitate transfer of referred patients are unable to be serviced by ContinuumRx to another home infusion provider if needed. Ensure a smooth and complete transfer and proactively communicate with the patient and MD office throughout the process. Capture pertinent information about all patient transfers for analysis. Act as the point of contact for home health agencies, MD offices and CRx staff for care coordination. Accepts responsibility to act as the Nursing Supervisor in the absence of the VP of Nursing. Provide direct oversight of the care coordination process offering advice and direction to team members (nurses and nurse schedulers). Complete understanding of confidentiality with respect to Company proprietary information as well as information concerning patient/client care; complying with all federal and state laws as apply to confidentiality of protected health information (PHI) and electronic protected health information (EPHI); and following HIPAA guidelines regarding readily identifiable protected health information Qualifications and Experience Graduate of a nursing program of study recognized by the State Board of Nursing employed in. Current, active and in good standing registered nurse license in the state of employment Experience administering infusion therapy in a home, hospital or clinical setting. Pediatric, ER, and/or medical surgical experience, a plus. Must have current CPR certification 3-5 years experience in homecare is preferred Must be able to lift 10 pounds Must possess a valid Drivers License May be required to operate a motor vehicle in adverse weather conditions
