and adolescent clients and 40hrs before working with adult clients.
Job Summary Psychiatric rehabilitation program (PRP) provides
rehabilitative and support services that assist individuals with a
severe behavioral health challenges so that they could attain a
higher level of independence. Services are individualized with a
goal to assist and patient(s)/client(s) to develop independent
community living skills, including how to manage their behavioral
health challenges while living in the community. Services may be
provided on-site at a PRP facility or off-site at an individuals
residence, job or other location in the community. On-site PRP
services provide a structured environment where rehabilitation
activities and services are provided predominantly in a group
setting. Individuals enrolled in a PRP program also receive: case
management services, including assistance with securing and
maintaining entitlement; transportation to appointments;
coordination of services and liaison with external services like
somatic, substance abuse and mental health practitioners.
Rehabilitation coordinators are supervised by Rehabilitation
Specialist, Rehab Associate and Program Director. Rehabilitation
Coordinator Responsibilities Advocates for patient(s)/client(s) in
the community Implement and assist the patient/client in meeting
the identified rehabilitation plan established in their
Individualized Rehabilitation Plan (IRP) Analyze information from
interviews, educational and medical records, consultation with
other professionals, and diagnostic evaluations, in order to assess
clients abilities, needs, and eligibility for services. Arrange for
physical, mental, academic, vocational, and other evaluations to
obtain information for assessing clients needs and developing
rehabilitation plans. Collaborate with clients families to
implement rehabilitation plans that include behavioral,
residential, social, and/or employment goals. Confer with clients
to discuss their options and goals so that rehabilitation programs
and plans for accessing needed services can be developed. Confer
with physicians, psychologists, occupational therapists, and other
professionals, in order to develop and implement client
rehabilitation programs. Develop and maintain relationships with
community referral sources such as schools and community groups.
Monitor and record clients progress in order to ensure that goals
and objectives are met. Prepare and maintain records and case
files, including documentation such as clients personal and
eligibility information, services provided, narratives of client
contacts, and relevant correspondence. Collaborate with community
agencies to establish facilities and programs to assist persons
with disabilities. Locate barriers to client employment, such as
inaccessible work sites, inflexible schedules, and transportation
problems, and work with clients to develop strategies for
overcoming these barriers. Participate in job development and
placement programs, contacting prospective employers, placing
clients in jobs, and evaluating the success of placements.
